We believe in transparency of rates. Couch & Cardigan accepts credit cards, checks, cash, health care savings cards, and debit cards. Payment is due at the time of service.

Initial evaluation (75 minutes): $400

Follow-up appointment (20 minutes): $200

Extended follow-up appointment (40 minutes): $350

Medication refills outside of scheduled appointments: $50

Other Professional Services (such as report writing, completion of forms, record review, or phone calls greater than 5 minutes): prorated at hourly rate: $400/hour

Forensic Consultations: prorated at hourly rate: $500/hour 

Your appointment is reserved just for you. If an appointment is not cancelled or rescheduled more than 24 business hours ahead of the scheduled start time, a ‘Did Not Keep Appointment’ (DNKA) fee will apply. Appointments are expected to start on time and end on time. Patients 10 minutes or later for an appointment, or patients who do not arrive for their appointment, will be considered as having missed the appointment and will be charged the DNKA fee. Stating that you did not receive a reminder does not exempt you from a DNKA fee.

Fees are subject to change and reflect the complexity and type of service(s) provided. You will be notified thirty days in advance of any changes in our professional fees.
